Services

Primary care, urgent care, and pain management — practiced the way they make sense to us.

Primary care

  • Annual physicals and comprehensive wellness exams
  • Chronic disease management — diabetes, hypertension, cholesterol, thyroid
  • Preventive care, cancer screening, and risk-factor counseling
  • Nutrition, weight, and exercise counseling
  • School, sports, and pre-employment physicals
  • Lab interpretation and ongoing medication management

Urgent care

  • Acute illness — respiratory infections, flu, COVID, strep, urinary infections
  • Minor injuries, sprains, and fractures
  • Laceration repair and wound care
  • Skin infections, rashes, abscess drainage
  • Cryotherapy for warts and skin lesions
  • Same-day evaluation instead of an emergency room bill

Pain management

An older man's hands resting on a walking cane
  • Musculoskeletal and joint pain evaluation
  • Joint and soft-tissue injections
  • Back and neck pain — conservative, function-first management
  • Sports injuries and return-to-activity planning
  • Non-opioid pain strategies and physical therapy coordination

Weight management

Physician-supervised weight care — a metabolic workup, a review of the medications that may be working against you, and prescription treatment when it is appropriate. This is medical care, not a program you buy.

House calls

Some visits should come to you. If you are recovering from surgery, caring for a parent, homebound, or simply too unwell to sit in a waiting room, I will bring the visit to your house. I spent years practicing in patients’ homes before opening this practice, and it remains the part of the work I find most useful.

Text or call to arrange one. House calls are generally scheduled outside regular clinic hours, including evenings and weekends. I travel throughout Upland and the surrounding communities — Rancho Cucamonga, Claremont, Ontario, Montclair, and La Verne. The fee depends on distance and time, and is quoted before the visit is booked, never after.

  • Post-operative and post-hospital follow-up at home
  • Homebound and mobility-limited patients
  • Acute illness when leaving the house is a bad idea
  • Evaluating an older parent in the setting where problems actually show up
  • Evening and weekend availability by arrangement

Serious illness and palliative support

Grandparents with two young grandchildren

This is not hospice enrollment, and it does not replace your oncologist or specialist. But if you or a family member is living with a serious illness, the practical questions — symptom control, what the next six months realistically look like, how to talk to the rest of the family, what paperwork actually matters — are ones I have spent my career answering.

  • Symptom management for advanced illness
  • Goals-of-care and treatment-decision conversations
  • Advance directives and POLST completion
  • Family meetings and caregiver support
  • Coordination with hospice or specialty teams when that time comes

What we do not provide

Being clear about this up front saves everyone time. This practice does not provide:

  • Emergency, hospital, or inpatient care
  • Surgical or operative procedures
  • Obstetric and gynecologic surgical care
  • Specialty consultative care (we refer, and we help you navigate it)
  • Chronic prescriptions for benzodiazepines, opioids, or ADHD stimulants

Two ways to be seen

Pay per visit

$150 per office visit. Nothing else required. Good if you need care now and do not want an ongoing commitment.

Membership

$99 per month for 24/7 access, care between visits, and refills handled without coming in. Good if you want a doctor, not just an appointment.

Ready when you are

Call or text (626) 552-6720 to book a visit, or start a $99/month membership and have a doctor in your corner every day of the year.
